I am trying something new for all Wendron Goats. Two ways to register.

EITHER
The registration form is available in PDF or WORD doc form below. Please download it and read it. If you want to print out the application form and hand it to me signed with you payment then that is great.
PDF Version – for printing and then filling in

Microsoft Doc Version – for filling out on your computer and then printing.
MS doc – membershipform 2011-2012

OR

An alternative is to down load the form, read all the conditions etc but fill in the form here. I can email it back to you for your records and also bring a printed copy along to training and you can sign it and hand over any payment.

DECLARATION
1. I understand that Wendron CC Youth Football Section, its servants, agents or employees are under no liability for loss
of property, accidents or injuries of any kind to my child/children, however caused during the course of training,
preparation or matches.
2. In the event that my child/children are injured whilst playing football/travelling to/from football events and I cannot be
contacted on the above numbers, I hereby give consent for my child to receive medical attention.
3. The club operates a website for the benefit of all associated with Wendron CC Football Club. At times images and
match reports will be uploaded for all to view. These images/reports will be solely used for the purpose they are intended
and will be removed from the site if requested by a parent/guardian.
